Lancashire Healthy Young People and Families Service

We began providing health visiting and school nursing across Lancashire in 2019, when we launched Lancashire Healthy Young People and Families Service, in partnership with Lancashire County Council.

Prior to this, services were delivered by two different providers in Lancashire and our job was to bring more than 500 colleagues together in to a single service focused on children, young people and families across the county.

Our aims are to:

  • Provide a service focused around children, young people and their families
  • Base all we do on evidence that delivers improved health and wellbeing outcomes
  • Deliver an integrated and collaborative service with Lancashire County Council’s Child and Family Wellbeing Service, especially at point of care
  • Embed health improvement and health promotion into the principle of Making Every Contact Count
  • Ensure information is timely and shared in an appropriate manner with professionals and families

What we've achieved

Since April 2019 we have:

• Brought together more than 500 colleagues from two previous providers in to one single service, and established three hubs in Preston (Central), Burnley (East) and Lancaster (North) as well as multiple spokes for colleagues to work from.

• Created a virtual Single Point of Access (SPA) which allows administration colleagues to work remotely and more efficiently when communicating with professionals and families. Our SPA team answered 32,000 calls over seven months in 2020.

• Introduced mobile working so that colleagues can work more flexibly and spend more time with patients instead of having to travel back and to from their office base.

• Developed a dedicated Looked After Children team to co-ordinate and carry out the best care for looked after children in Lancashire.

• Launched virtual groups for parents and carers to access advice on a range of topics as an alternative to attending face-to-face appointments and sessions. More than 350 parents and carers have already benefited from our these groups, launched in 2020.

• Begun close working with Lancashire County Council’s Child and Family Wellbeing Service so we can develop an integrated working approach for the benefit of Lancashire's families.

• Led on an assessment that saw LHYPFS’s infant feeding team, Lancashire County Council's Children and Family Wellbeing Service and Families and Babies (FAB) receive the gold standard from UNICEF’s Baby Friendly Initiative.

Our vision

We will continue to work in Lancashire to deliver our bid promises and considerable additional value to commissioners, the community and our colleagues.

A visionary, driving influence across the region, we want our communities to be our champions and our patients, partners in their own care. We will continue to positively engage and be visible with stakeholders. Our colleagues will be recognised as transformation champions.

We will strive to achieve outstanding CQC inspections and continue to bring innovation and inspiration to our services.
